After you have scheduled your first appointment you will receive an email confirming the date and time.  To make the most of your time, it is very helpful if you can fill out the new client forms and email them to me before your appointment.  You can download the forms below.  You can also find directions to my office below. Please bring in your insurance card if you wish to file claims and any other relevant records. Additionally we will keep a credit card on file for copays and balances.  I look forward to meeting you.  Please call me if you have any questions.
